I understand and agree to make monthly installment payments of $240.00 per month by credit card and subsequent installments on the same day (1st day of the month) of each consecutive month until paid in full, and that the monthly installments are to be paid to All-Pro Tae Kwon Do Academy by credit card only through auto pay (Gymdesk). Checks not accepted. The monthly installment payments will be due on the 1st day of each month and no later than the 5th day of the month. A Late Fee of $25.00 will be billed if the monthly installment payments are received on the 6th day of the month and beyond or if the credit card gets rejected. Monthly Tuition is increased once per calendar year (January) by a fee of $10 (5%) per month, per child. Grandmaster Andrew Fanelli and his Black Belt staff instructors supervise all classes. Being a student of All-Pro Tae Kwon Do Academy, I will faithfully comply with all the rules and regulations of the Academy and the traditions of Tae Kwon Do. Failure to take the classes in the allocated time, without written approval from Grandmaster Andrew Fanelli, invalidates the classes beyond the expiration date. Classes are not conducted on Sundays, national holidays, or testing days. All-Pro Tae Kwon Do Academy doesn't offer refunds, pro-rations, extensions, or credits. I understand further that strict observation by me of the rules and regulations related to my training or my children’s training, including the use of protective equipment required by the Academy (sparring gear, staff, sai, etc), will largely eliminate the possibility of accidents or injuries, and I hereby represent that I’m or my children’s physically fit to take the prescribed course of instruction and that I have had an opportunity to observe and participate in the art of Tae Kwon Do prior to signing of this agreement. All equipment (sparring gear, staff, sai, etc) must be ordered through the studio only, no exceptions. I further understand and agree that All-Pro Tae Kwon Do Academy will not be held liable for any injuries, damages, etc, incurred in the Academy by or resulting from the negligence of the owners, operators, or persons in charge of such establishment, or their agent, servants or employees.
I acknowledge reading & receiving a copy of this agreement. A 30-day advance notice (first day of the month - example: 2/1, 3/1, 4/1, etc) by email is required for cancellation, no exceptions.
